Motor Vehicle Collision Requests (MVC)

HOW TO MAKE A REQUEST

Requests for motor vehicle collision reports can be made through the Freedom of Information Request route (download). See the Freedom of Information tab on our website for more information.

You can mail the request form or attend your local NAPS detachment.  All requests must be accompanied by the appropriate payment and ID. MVC reports will not be given out at the detachment.

INSURANCE COMPANIES

Insurance Companies may request a Motor Vehicle Collision Report (download) by mail.  The request must be made in writing, and be accompanied by a signed consent form, and company cheque, made payable to Nishnawbe Aski Police Service in the amount of $5.00.

In order for an insurance company to be entitled to a report, there must be a connection between their insured and a person named in the report. Requests from insurance companies must be accompanied by a signed consent form from their insured. Please be aware that all 3rd party information will be redacted.

RESIDENTS WHO RESIDE IN A COMMUNITY POLICED BY NAPS

Individuals can attend the NAPS detachment in person, with two pieces of identification.  The officer will complete the request form, accept the appropriate payment (Cash or Money Order), and provide the individual with a receipt.

The individual making the request must be named as an individual within the report in order to receive a confirmation letter/report.  If an individual is making a request on behalf of a company, proof of employment with the company is required. 

NON-RESIDENTS

In circumstances where an individual cannot physically attend NAPS headquarters or a NAPS detachment due to distance, requests can be received through the mail.  A copy of the FOI request form (download) providing full details of the collision is required, along with a copy of photo ID.  A certified cheque or money order, payable to Nishnawbe Aski Police Service in the amount of $5.00 must accompany the letter of request. Mail all needed documents and payment to your nearest Regional Office:
